Read the form on the link you posted you could have logged in a vita once before and not even own one at all today

Did you guys even see the form. You don't have to offer any proof that you ever even owned a vita at all. It basically says "Do you promise you bought a vita before this date? Do you promise you haven't traded it in yet?"

To make sure, I checked my account to see if I did have my Vita before the deadline. I have records of me downloading games,apps and demos for Vita in May 2012. I knew that I bought my Vita around that time but I wasn't sure.

That junk says "a side panel of the PlayStation Vita package " are they talking about the original box . Funny thing is I usually keep EVERYTHING and the original Vita box is the one box that I was looking for recently and couldn't find.

they should have sent you a web form if you qualify. in that, the serial number section is optional.
