Sprint more, and stop throwing down health packs when you're NOT behind cover. Also, stop wasting your nades. I was watching you just chuck them for no reason at all. I looked at your radar and there wasn't anybody around where you were throwing them.
Also, start hitting R1 if you don't know where people are, you might just end up spotting somebody. You started chucking nades and there was a sniper on the roof top in your sights that I guess you didn't see at all. He ended up killing you and you easily could have known where he was at if you just clicked R1 a few times to survey the area.
