Being someone who dealt with something quite similar all i can tell him is that he needs to let her go.
This shyt not only is nasty but this is a big sign of emotional immaturity and also lack of awareness on her part. I don't know if she is insane but that is serious passive aggressiveness....
also she could be trying to hide a visible STD.My ex was also not washing to do this....and she also had bad hygiene habits.
He needs to get divorced BEFORE she has children by him but judging by the body odor and lack of sex I don't think this is a huge worry.

That's...not normal. There is likely a psychological or emotional trauma issue at play here, and I hope it gets resolved for both of their sakes.
