There are signs but from what I read it's similar to passing out. When you feel the chest pains it's too late. The warning signs are there before hand like sweating, lightheaded, arm or back pain but those don't feel like an incoming heart attack so people don't know. If you're aware of the symptoms you can try to chew an aspirin when you feel it and it might save you.
