1 clean loss. He did lose a lot of house show matches via count out or DQ to set up rematches around the horn. The general formula for him (and Bruno) seemed to be "Lose first match via count out, lose second via DQ, win in a standard match or, if heated enough, in a cage. Although in Bruno's case, he seemed to lose most of his cage matches by knocking his opponent through the door and then winning a straight match later.
The only matches he lost by pinfall in WWF 1983-1993
Andre in 1988 via second ref shenanigans, match was overruled and title vacated
Warrior at WM6, completely clean
Taker at Survivor Series 1991, after a tombstone on a chair (and assist from Ric Flair), won title back days later, vacated anyway
Yoko KOTR 1993 after a camera exploded in his face
As a comparison, Hogan had 4 clean pinfalls in 2002 alone. I think he had 10 singles matches in WCW where he actually lost by pinfall or submission. 7ish were clean.
