Superficially the term stems from the mountain range/region, but it soon became a catch-all for peoples with shared physical characteristics (particularly when phrenology was considered legit - skull shape). Europeans, some North African peoples, Middle Easterners, some Central Asians and Indians all fall in this category.

Day-to-day use in North America? It's been re-purposed mainly to describe visibly "white" people.
